Dr. Alter specializes in female genital surgery and has spent decades perfecting his reconstructive techniques. All patients featured in this gallery are unique and an individualized treatment plan was developed for each case.
20-year-old with radical amputation of her entire labia minora from a trimming labiaplasty. She was the first patient ever to undergo labia minora reconstruction with clitoral hood flaps, which was invented by me to correct her problem. She also underwent a clitoropexy with hood length reduction, and later had a minor revision to give her this good result.

47-year-old who had a radical amputation of her entire labia minora from a trimming labiaplasty. She underwent reconstruction of her labia minora with clitoral hood flaps.
a. Pre-operation b. Post-surgery
38-year-old who had a trimming labiaplasty with amputation of her right labium minora. She had minimal residual left labium with cross-hatching suture lines. She was reconstructed with removal of the minimal left labium and placement of clitoral hood flaps on each side.
a. Pre-operation b. Post-surgery
31-year-old who had a trimming labiaplasty with excision of her prepuce resulting in removal of the upper 2/3 of her right labium, a small irregular, scarred left labium, and over-exposure of her clitoris. She was reconstructed with clitoral hood flaps on each side with removal of the scarred left labium, a right lower “Alter V” wedge excision, closure of the hood over her clitoris, and excision of redundant perineal tissue.
a. Pre-operation b. Post-surgery
27-year-old with amputation of both labia minora from a trimming labiaplasty. She was reconstructed with clitoral hood flaps.

41-year-old with amputation of both labia minora from a trimming labiaplasty. She was reconstructed with clitoral hood flaps and a modified perineoplasty.
a. Pre-operation b. Post-surgery
23-year-old with amputation of both labia minora from a trimming labiaplasty. She was reconstructed with clitoral hood flaps.
a. Pre-operation b. Post-surgery
38-year-old who had a labial tear in childbirth. Her gynecologist treated it by amputating both labia. She was reconstructed with clitoral hood flaps.
a. Pre-operation b. Post-surgery
27-year-old who had a trimming labiaplasty resulting in amputation of both labia minora. Despite having a very small clitoral hood, her labia minora were reconstructed with clitoral hood flaps.
a. Pre-operation b. Post-surgery
39-year-old who had a trimming labiaplasty resulting in amputation of her left labium and a hanging frenulum on both sides. She also disliked her long clitoral hood. She underwent a clitoropexy with decreased protrusion of her hood, reattachment of her frenula to the labia, a left clitoral hood flap to reconstruct her left labium, and advancement of redundant lower right labium.
a. Pre-operation b. Pre-operation with an oblique view c. Pre-operation with labia open d. Post-operation e. Post-operation with an oblique view f. Post-operation with labia open
19-year-old who had a left trimming labiaplasty resulting in amputation of the lower half of the labium. She underwent reconstruction of her left labium with a modified left clitoral hood flap and reduction of her right labium with an “Alter V” wedge labiaplasty to achieve symmetry.
a. Pre-operation b. Pre-operation with labia open b. Post-surgery d. Post-surgery with labia open
39-year-old who had amputation of the mid-portions of both labia minora from a trimming labiaplasty. She had reconstruction of her right labium with advancement flaps and closure and reconstruction of her left labium with a left clitoral hood flap.
a. Pre-operation b. Pre-operation with labia open b. Post-surgery d. Post-surgery with labia open
55-year-old who had partial amputation of her right labium and scalloping with irregularity of her left labium. She underwent a right clitoral hood flap to reconstruct her right labium and two wedge excisions on the left to give her symmetry and to eliminate most of the scalloping.
a. Pre-operation b. Pre-operation
24-year-old who had a clitoral hood reduction with a trimming labiaplasty resulting in amputation of the left labium and near amputation with scalloping of her right labium. She underwent reconstruction of both labia minora with clitoral hood flaps. Also, various excisions and skin advancements were used to improve the attachments of the clitoral frenula to the new labia minora and to improve the bottom of the retained labia. The short clitoral hood limited the lower length of the reconstructed right labium. The left had some residual lower labium.
a. Pre-operation b. Pre-operation with labia open b. Post-surgery d. Post-surgery with labia open
63-year-old who had a trimming labiaplasty with labial amputation, clitoral enlargement, and an open perineum. She had a two-stage reconstruction with clitoral hood flaps to reconstruct her labia minora along with vaginal tightening followed up by a clitoral reduction and fat injections to her labia majora.
