CLITOROPEXY/CLITORAL HOOD REDUCTION
The clitoral hood may protrude too much causing the woman to be self-conscious or irritated. The protruding hood and clitoris causes a bulge in her clothing or the appearance of a small penis. In order to achieve synchrony, many women also desire clitoral hood reduction to be in balance with smaller labia minora.
Many options are available to reduce the clitoral hood. If there is excess width, I routinely reduce the sides of the clitoral hood while performing labiaplasty. I first reported this in 2008 as the extended hockey stick reduction. If the hood is overlapping the clitoris, it can sometimes be shortened by only excising skin horizontally. If the hood is not long but thick and protruding, the tissue under the skin can be thinned out.
However, it is often necessary to change the length, width, or projection of the clitoral hood. In order to resolve this situation, I often make a horizontal incision on the lower clitoral hood and reposition the clitoris slightly higher and deeper with a few sutures (clitoropexy). This pulling up and moving in of the head of the clitoris eliminates the protrusion and now allows removal of some longitudinal hood skin. The hood becomes shorter and narrower with elimination of over-protrusion of the hood and clitoris. There is no risk of nerve injury or sensation change.
The clitoropexy can be done alone or in combination with the Alter Central Wedge Labia Minora Reduction.
